Metacognitive Training for Depression (D-MCT) (Japanese)

うつ病のためのメタ認知トレーニング(D-MCT)は、うつ病の治療のため新しいコンセプトに基づいた技法です。これは、認知行動療法(CBT)の一形態として概念化したもので、非機能的な信念の変容に焦点を当てられるようメタ認知的視点を取り入れています。トレーニングでは、グループの参加者が、うつ病に伴いよく生起する自動的で非機能的な思考パターンをその思考プロセスを理解することにより(個人的解釈から離れる)、思考パターンを認識し、修正できるようになることを目的とします。うつ病的思考を止めるため、参加者に、生活の中から実践的で理解しやすい例を基にうつ的思考に対し、クリエイティブでアプローチのしやすい方法を伝えるようにしています。また、非機能的対処法(例:思考抑制、問題解決としての反すう)に加え、ひとの考えるプロセスについての非機能的思い込みにも焦点を当てています。トレーニングは、精神病のためのメタ認知トレーニング(MCT:www.uke.de/mct)にヒントを得てはいますが、それとは違ったうつ病特有の認知のゆがみを取り扱っています。

トレーニングのベータ・バーションの実現可能性と効果は、出版されているパイロット研究(Jelinek, Otte, Arlt & Hauschildt, 2013)により確認されました。トレーニングは、うつ症状の軽減に効果があることが中等度のエフェクト・サイズが出たことでわかりました(Cohen’s d = 0.56-0.73)。また、認知バイアスと反すうが有意に下がり、自尊心が回復しました(エフェクト・サイズ d = 0.26 – 0.64)。

我々は、研究結果をトレーニング内容の向上のため活用しました。最新のRCT研究においては、身体トレーニング(ウォーキング)との比較を行いました。すべての患者さんは、ハンブルク・リハセンターで外来患者用標準心身治療プログラムを受けました。6ヶ月フォローアップ後、D-MCTを受療した患者さんのうつ症状が、身体トレーニングを受けた人たちよりも大きく改善しました(Jelinek et al. 2016).
